As a patient and under the General Data Protection Regulation you have the right to access your Healthcare Record held by Maidstone and Tunbridge Wells NHS Trust.
To access a Healthcare Record a request is required to the Subject Access Request (SAR) department; if you wish to make a request the following items are required to disclose the information:
1. Completed Subject Access Request for Healthcare Records
2. Documentation (if required) and Proof of identity (detailed in the Subject Access Request for Healthcare Records form)
Once the form, documentation (if required) and proof of identity have been received by the department you will get confirmation by post or email and the department will begin the process to obtain and copy the requested records.
While we make every effort to provide information to those who request it there are certain circumstances where this is not possible i.e. destruction. Should this effect your request we will endeavour to keep you informed.
Upon receipt of your completed request we aim to disclose your records to you within the maximum deadline of 30 days (if additional clarification is needed, the 30 day period does not start until that is received).

Health Records workshops and drop-in sessions
The workshops are designed to provide a general guidance/overview of patient Healthcare records, clarity of administration errors that can affect patient care and the correct use of Allscripts to support this; to enable users to create, view, track and request Healthcare Records.
The training is for new staff to the Trust and anyone wishing a refresher involved in the administration and management of patient records, namely Admin and Clerical.
Space is limited and booking is required.
