What we do
We provide a specialist service to patients who have been diagnosed with a life limiting illness, their relatives and those important to them.
We work closely with doctors and other members of the team such as physiotherapists, occupational therapists and the integrated discharge team to help with some of the wider issues associated with illness.
Maintaining close working links with the local hospices also helps us provide a seamless service and promote continuity of care.
We aim to:
- Act as a resource to ensure dignified, individualised end of life care centred around the needs and preferences of the patient and their family
- Identify and relieve physical symptoms, helping with rehabilitation and wellbeing
- Facilitate open and honest communication
- Identify and relieve social, psychological and spiritual difficulties
- Improve quality of life for patients.

Our team
We work across Maidstone and Tunbridge Wells hospitals, and our team includes:
- Two Palliative Medicine consultants
- Lead Nurse for Palliative and End of Life Care
- Macmillan palliative care clinical nurse Specialists
- Two Macmillan cancer support workers
- Palliative Care Support Secretary
- Consultant medical support from local hospices.
Referrals to the team can be made by any professionals involved in the patient's care.
