As a family member, partner, friend or carer of someone who has died while in our care, you may have feedback about the care and treatment they received.

Our aim is always to provide excellent end of life care for patients and for those important to them. Our bereavement survey is for you to share your experience of the care we provided.

We understand this may be a very difficult time for you, and participation in the survey is voluntary. Please do not worry if you are not ready to take part straight away.  If you would prefer to respond at a later date you can, even if it’s some weeks or months following the bereavement. If and when you feel ready to share your experience, your feedback will help us improve our services.

Please only complete the survey once, and you are welcome to share the link with others who may want to give feedback too.

The survey is designed to maintain your anonymity (you won’t need to provide any personal information) so we can’t respond to individual comments.
