b) Of those cases, how many were disciplined/dismissed following an investigation into the individual AND/OR surgical unit by one of these bodies i) Royal College of Surgeons, ii) GMC iii) CQC?
c) For each of these, please provide details of how many serious incidents occurred and the details of each incident.
Note: By ‘serious incident’, I am referring to the definition and description of a serious incident outlined in the NHS England Serious Incident Framework.
