2. The total number of sites under the management of your Trust during the period above. If your trust has merged or restructured during the period please state the date and the names of the organisations involved.
Please supply the total figure for recorded Serious Incidents during the stated period relating to ALL sites/operations under the management of your Trust. It is NOT necessary to supply a broken down figure for each site.
