The information we would like is your organisational chart or structure as it applies to the maternity services that you provide.

In particular we are interested in the lines of management and accountability that exist between heads & directors of midwifery and the highest levels of management within your trust (including at board level) and posts to which heads & directors of midwifery report.

Please also include all roles that sit within the maternity services you provide including specialist midwife roles (e.g. smoking cessation specialist midwives) and any other posts not within maternity that report to heads & directors of midwifery.
