• Login External users  (restricted to users within COIN (Community of Interest))

Login (External Users)

Type http://10.136.96.138/telepath in address bar of internet explorer. The following screen should appear

Enquiry - click on "open in new window" to launch the screen below

Type in user ID in user name and password. The screen below should launch

Fill in details in boxes as appropriate

Reset Forgotton Password

Send email to mtw-tr.pathology@nhs.net

Request new account

Please print this form, complete it and either

• scan and return to mtw-tr.pathology@nhs.net

or

• post to Pathology IT manager, Microbiology Department, Maidstone Hospital, Hermitage Lane, Maidstone ME16 9QQ
